I met with PA Lana Formo, as a result of a scheduling mishap, on 2/13/25. I was a bit trepidatious, as meeting with someone other than Dr. Eric Cornidez, in the past… proved to be a harrowing experience.

To my pleasant surprise, Ms. Formo proved to be an exceptionally professional and compassionate provider. I did not have to regurgitate years of my painful migraine history, in order for her to fully grasp that I utilized my opioid medication only as needed and only as prescribed. She asked relevant questions, listened carefully, and shared information that clarified her complete understanding of my personal journey with chronic migraine pain.

I left my appointment with PA Formo, feeling both relieved and impressed. PISA is fortunate and blessed to her on their provider staff.

Upon my departure today, I thought to myself…PA Lana Formo is the female counterpart of Dr. Eric Cornidez…and that is the HIGHEST compliment she can receive from a patient of PISA.

Thank you Noel for being the best asset to Dr Duran your the best and care for patient’s and their needs. Thanks for listening and caring.

I like the professionalism and politeness of the staff. The offices are very clean. They really seem to care about your needs and want to really help you.

I can’t say enough good things about Lana. She listens and is so attentive to my needs. There is a warmth about Lana that sometimes is hard to find in the medical field. Plus she is extremely knowledgeable about the human body. Highly recommend!

Great first impression meeting Dr. Cubillo. Knowledgeable, explained in detail about plan for my pain issue. Showed interest in helping me relieve my pain and scheduled me ASAP for first treatment. Office staff very good.
